Being in the hospital can be frightening. Sometimes, coming home can be frightening, too. As a patient, we’re often afraid that we’re not ready. As a caring family member or friend, we’re anxious that we won’t know how to help. The discussion will focus on proven pre- and post-discharge strategies that will increase your confidence and your ability to continue your recovery at home and to avoid unwanted re-hospitalization.
Christine Damon is a gerontologist and adult educator. Also a four-time family elder caregiver, she combines personal experience with professional expertise in geriatric care management, care transitions, dementia care and training, adaptive housing, discharge planning, and caregiver support. Chris holds Master's degrees in Adult Education and Gerontology.
